If you have any questions about our optometry and ophthalmology eye care services, please contact the location nearest you. You can also use our secure online appointment request form to schedule your next visit with us.

South Florida Vision offices are located throughout South Florida and can be found on the following pages:

  • Optometry Locations (eye exams, glasses/contacts, etc.)
  • Ophthalmology Locations (cataracts, glaucoma, macular degeneration, etc.)

What Else You Need to Know

Office Hours

Unless otherwise noted at a specific location, our regular office hours are Monday – Friday, 8 am – 5 pm.

Medical Emergencies

In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT!

After Hours Urgent Medical Needs

If you have an urgent medical need and need to talk to one of our doctors after our regular hours, please call the South Florida Vision location nearest you. A doctor is on-call 24 hours a day. Your call will be returned promptly.

Insurance / Payment / Billing

At South Florida Vision, we accept Medicare and most major insurance plans. You can review a list of current insurances we accept for our optometry and ophthalmology eye care services. If your insurance plan is not on these lists, please contact your insurance company or you may call the South Florida Vision location nearest you to find out if we accept your plan.

If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, Discover, American Express, and Care Credit.

If you have a question related to your bill or insurance, please contact the South Florida Vision location nearest you.

Referrals

If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an ophthalmology or optometry office visit.

Please coordinate this with your primary care physician in advance. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit.

Hospital & Surgery Center Affiliations

Surgeons at South Florida Vision have hospital or surgery center affiliations with:

  • Boca Raton Surgery Laser Center
  • Eye Surgery Center in Fort Lauderdale
Prescription Requests

Prescriptions and refills for established patients of South Florida Vision are routinely issued during our regular office hours (see above). You can make your request by contacting the South Florida Vision optometry location or ophthalmology location nearest you.

Refills are generally not issued during the evenings, weekends, or holidays, as your doctor may not have access to all your medical records during those times. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy.
